While pharmaceutical industry involvement in producing, interpreting, and regulating medical knowledge and practice is widely accepted and believed to promote medical innovation, industry-favouring biases may result in prioritizing corporate profit above public health. Using diabetes as our example, we review successive changes over forty years in screening, diagnosis, and treatment guidelines for type 2 diabetes and prediabetes, which have dramatically expanded the population prescribed diabetes drugs, generating a billion-dollar market. We argue that these guideline recommendations have emerged under pervasive industry (...) influence and persisted, despite weak evidence for their health benefits and indications of serious adverse effects associated with many of the drugs they recommend. We consider pharmaceutical industry conflicts of interest in some of the research and publications supporting these revisions, and in related standard-setting committees and oversight panels. We raise concern over the long-term impact of these multifaceted involvements. Rather than accept industry conflicts of interest as normal, needing only to be monitored and managed, we suggest challenging that normalcy, and ask: what are the real costs of tolerating such industry participation? We urge the development of a broader focus to fully understand and curtail the systemic nature of industry’s influence over medical knowledge and practice. (shrink)

Jewish Agrarian Movements (JAM hereafter) in North America express the many different shapes and iterations of Jewish farming on the continent, grounded in historical perspectives that influence current practices and activities. From within this diversity, common threads emerge with much to contribute to agrarian social movements and scholarship. Jewish values of returning (_t_’_shuvah_), releasing (_shmitah_), and repairing (_tikkun_), along with theories of _doikayt_ (an anti-zionist movement around “hereness”) and radical diasporism, animate JAM’s critical engagement with agri-food systems. As researchers who (...) have both studied and participated in Jewish agrarianism in a variety of U.S. and Canadian contexts, we solidify a series of themes and tensions that emerge from JAM: diaspora and indigeneity, modernity and tradition, Jewish agroecological knowledge production, and lived religion. We argue that, while JAM has not yet been examined thoroughly within critical food scholarship, it has the potential to contribute to broader debates and frameworks within sub-fields such as radical food geographies, critical agrarianism, and decoloniality. Without consideration of JAM as a part the study of food and agriculture, there are risks of marginalization of farmers, activists and researchers of Jewish identity. (shrink)

This paper draws upon Hannah Arendt's idea of the 'right to have rights' to critique the current protection gap faced by refugees today. While refugees are protected from refoulement once they make it to the jurisdiction or territory of a state, they face an ever-increasing array of non-entrée policies designed to stymie access to state territory. Without being able to enter a state capable of securing their claims to safety and dignity, refugees cannot achieve the rights which ought to (...) be afforded to them under international law. Drawing upon both legal theory and political philosophy, this paper argues that refugees today, just as the stateless in Arendt’s time, must be afforded the ‘right to have rights’, understood as a right to enter state territory. ABSTRACTThis paper examines the differences between the thought of Hannah Arendt and Emmanuel Levinas concerning the “Rights of Man”, in relation to stateless persons. In The Origins of Totalitarianism, Arendt evinces a profound scepticism towards this ideal, which for her was powerless without being tethered to citizenship. But Arendt’s own idea of the “Right to have Rights” is critiqued here as being inadequate to the ethical demand placed upon states by refugees, in failing to articulate just what states might (...) be responsible for. I argue that the ethical philosophy of Levinas meets this lacuna in Arendt’s thought, via his concept of the Face as the locus of human dignity and to which states can be recalled to responsibility. Levinas wrote several papers on what he called “the phenomenology of the Rights of Man”, and in his phrase, which provides a summation of precisely what is lacking in Arendt’s arguments: “In the face – a right is there”. Referring to Hannah Arendt’s book Eichmann in Jerusalem, the Southern US fiction writer Flannery O’Connor expressed the effect of the revelations about the horrors of Nazi Germany as ‘haunting’. Taking this comment and her admiration of Arendt as a cue, this article rereads Flannery O’Connor’s fictional depiction of secular characters. Usually lauded or critiqued for her entanglement in ‘otherworldly’ concerns, here these concerns become comprehensible as much as political intervention as motivated by ‘religious’ belief. O’Connor’s frequently humorous use of (...) her fiction as a retort to the secular world was inflected by her reading of Eric Voegelin’s contemporary secularization thesis with its criticism of all ‘isms’. In this context, O’Connor’s admiration for Arendt becomes all the more intriguing (since Arendt’s interpretations of the human condition clashed with Voegelin’s), and allows one to stage a theoretical meeting in order to explore O’Connor’s depiction of the secular in relation to a speculative exploration of how Arendt might have responded to the fiction of O’Connor. Such a staging is accomplished here via a reading of O’Connor’s short story ‘The Lame Shall Enter First’ read against Arendt’s concerns, principally those expressed in The Human Condition. (shrink)

In many communities around the world, speech to infants and small children has increased mean pitch, increased pitch range, increased vowel duration, and vowel hyper-articulation when compared to speech directed to adults. Some of these IDS and CDS features are also attested in foreigner-directed speech, which has been studied for a smaller range of languages, generally major national languages, spoken by millions of people. We examined vowel acoustics in CDS, conversational ADS, and monologues directed to a foreigner in the Towet (...) dialect of the Papuan language Nungon, spoken by 300 people in a remote region in northeastern Papua New Guinea. Previous work established that Nungon CDS entails optional use of consonant alteration, special nursery vocabulary, and special morphosyntax. This study shows that Nungon CDS to children aged 2;2–3;10 lacks vowel hyper-articulation, but still displays other common prosodic traits of CDS styles around the world: increased mean pitch and pitch range. A developmental effect was also attested, in that speech to 2-year-olds contained vowels that were significantly longer than those in speech to 3-year-olds, which in turn had vowels of similar duration to those in Nungon ADS. We also found that Nungon FDS vowel triangles, measured from monologues primarily directed to a non-native speaker, were significantly larger than those of either CDS or conversational ADS, indicating vowel hyper-articulation. The Nungon pattern may align with the patterns of vowels in Norwegian IDS, CDS, and FDS, where hyper-articulation is found in FDS, but not CDS or IDS. The languages of the New Guinea area constitute 20% of the world's languages, but neither an acoustic comparison of vowels in CDS and ADS, nor an acoustic study of FDS, has previously been completed for any language of New Guinea. The function of an FDS style in a small, closed community like those of much of New Guinea may differ from that in larger societies, since there are very few non-native speakers of Nungon. Thus, this study uses monologues recorded with a foreign researcher as interlocutor to study Nungon FDS. (shrink)

BackgroundThere are numerous ethical challenges that can impact patients and families in the health care setting. This paper reports on the results of a study conducted with a panel of clinical bioethicists in Toronto, Ontario, Canada, the purpose of which was to identify the top ethical challenges facing patients and their families in health care. A modified Delphi study was conducted with twelve clinical bioethicist members of the Clinical Ethics Group of the University of Toronto Joint Centre for Bioethics. The (...) panel was asked the question, what do you think are the top ten ethical challenges that Canadians may face in health care? The panel was asked to rank the top ten ethical challenges throughout the Delphi process and consensus was reached after three rounds.DiscussionThe top challenge ranked by the group was disagreement between patients/families and health care professionals about treatment decisions. The second highest ranked challenge was waiting lists. The third ranked challenge was access to needed resources for the aged, chronically ill, and mentally ill.SummaryAlthough many of the challenges listed by the panel have received significant public attention, there has been very little attention paid to the top ranked challenge. In this age of debate it is not news that what constitutes 'truth' is often at issue in environmental debates. But what is often missed is an insight that the speakers of Middle English understood a millennium ago: that truth comes from trust, which, is the central theoretical position of this paper. Our point is that truth depends essentially on social relations – relations that involve power and knowledge, to be sure, but also identity. Thus, challenges to what constitutes the (...) 'truth' are equally challenges to identities and the social networks of trust in which that truth is embedded. We therefore attempt to move beyond Foucaultian discursive theory by reintroducing the subject as both the product and producer of discourse. For Foucault, the subject is reduced to the discursive relations of power/ knowledge. In his effort to free us from the Cartesian cogito and the modernist absolutisms that eventually followed, Foucault lapses into a kind of postmodern functionalism. We argue that we should not speak of power/ knowledge, as Foucault suggested, but of power/ knowledge /identity, recovering the actors and concrete social relations that produce discourse, and are not only produced by it. We then argue that these social relations become constituted in particular moments of phenomenological challenge – discursive moments that confront the existing social relations of knowledge and their dialogue of trust and truth. We illustrate the implications of a threat to the social relations of environmental knowledge through an analysis of one such moment of phenomenological challenge: a dispute over whether or not the power plant in the community where we used to live, Ames, Iowa, is producing dioxin. Robert Coles' sentiment characterizes well the moral tenor of medical education today. Indeed, medical educators are frequently “seized by spasms of genuine moral awareness,” as they try to cope with the massive social and economic problems that face medical schools and teaching hospitals. The perception among educators that we currently fail to adequately teach several core aspects of doctoring, including professional values and behavior, constitutes one such spasm. In this case, the proposed remedy has generated considerable enthusiasm, but whether the (...) “core competencies” curriculum will make a difference, or simply “accommodate to the prevailing rhythms of the world we inhabit,” remains to be seen. Entomophagy—the practice of eating insects—has been touted as a means to combat undernutrition and food insecurity globally. Insects offer a nutritious, environmentally friendly alternative to resource-intensive livestock. But the benefits of edible insects cannot be realized if people do not choose to eat them. We therefore examine the social acceptability of edible insects in rural Zambia, where entomophagy is common but underexplored. Through a village case study, we show that edible insects are not valued equally, are understood socially, and seem (...) to reflect and reinforce social values. We utilize grounded theory and ethnographic methods, including semi-structured interviews, focus groups, and observation to examine collective entomophagy beliefs. While we expected to see differentiation in perceptions across groups based on kinship, we demonstrate that social values related to class, urbanism, gender, and age emerge as more germane explanations for entomophagy perceptions, reflecting their social weight. By expanding on current apperception of entomophagy behavior, our findings inform future research and efforts to promote entomophagy through minilivestock farming. Systems designed to maximize output, minimize labor, and highlight benefits are more likely to be widely accepted. We do not anticipate tribal association will be the primary limitation on minilivestock adoption in this context. (shrink)

In the conclusion to this multi-part article I first review the discussions carried out around the six essential questions in psychiatric diagnosis – the position taken by Allen Frances on each question, the commentaries on the respective question along with Frances’ responses to the commentaries, and my own view of the multiple discussions. In this review I emphasize that the core question is the first – what is the nature of psychiatric illness – and that in some manner all further (...) questions follow from the first. Following this review I attempt to move the discussion forward, addressing the first question from the perspectives of natural kind analysis and complexity analysis. This reflection leads toward a view of psychiatric disorders – and future nosologies – as far more complex and uncertain than we have imagined. (shrink)

We agree with Grossmann that fear often builds cooperative relationships. Yet he neglects much extant literature. Prior researchers have discussed how fear (and other emotions) build cooperative relationships, have questioned whether fear per se evolved to serve this purpose, and have emphasized that human cooperation takes many forms. Grossmann's theory would benefit from a wider consideration of this work.

In face of the multiple controversies surrounding the DSM process in general and the development of DSM-5 in particular, we have organized a discussion around what we consider six essential questions in further work on the DSM. The six questions involve: 1) the nature of a mental disorder; 2) the definition of mental disorder; 3) the issue of whether, in the current state of psychiatric science, DSM-5 should assume a cautious, conservative posture or an assertive, transformative posture; 4) the role (...) of pragmatic considerations in the construction of DSM-5; 5) the issue of utility of the DSM – whether DSM-III and IV have been designed more for clinicians or researchers, and how this conflict should be dealt with in the new manual; and 6) the possibility and advisability, given all the problems with DSM-III and IV, of designing a different diagnostic system. Part 1 of this article took up the first two questions. Part 2 took up the second two questions. Part 3 now deals with Questions 5 & 6. Question 5 confronts the issue of utility, whether the manual design of DSM-III and IV favors clinicians or researchers, and what that means for DSM-5. Our final question, Question 6, takes up a concluding issue, whether the acknowledged problems with the earlier DSMs warrants a significant overhaul of DSM-5 and future manuals.
